Our fees are based on the type and extent of treatment you require. If you provide us with your current medical and dental insurance information and cards, we will file your claims for you. Surgeries will be pre authorized and estimates of co-payments, deductible and balances due will be advised. All financial arrangements will be openly discussed at the consultation visit. Payment arrangements may be made if necessary, and we accept MasterCard, Visa, and Discover for your convenience.

Please advise us of any changes in your address, health status, phone number or insurance.

Our office participates with most MAJOR insurances. However, it is advised that you contact our office for a current status of our insurance participation.

Please note that any deductible, copays, non-covered services and balances due are the responsibility of the patient at the time of service.
